When completed, fax the forms to the attention of “Carrier Compliance” at 972.271.4648. Be sure to include your Certificate of Liability and Cargo Insurance (Quest Logistics shown as a certificate holder), Operating Authority, and W-9.
Once you are approved, you can begin hauling loads for Quest. You can request a username and password that enables you to view available loads and check payment status for your invoices. For more information, call 214.275.1080.
